When you register as a patient of our practice, you provide consent for our dental practitioners and practice staff to access and use your personal information so that we can provide you with the best quality of care and manage your dental health. Only staff who need to see your personal information will have access to it.  If we need to use your information for anything else, we will generally seek additional consent from you to do this, unless obtaining your consent is not practical or reasonable, and an exception under the Privacy Act 1988 (Commonwealth) applies.

We may collect your personal information when you register as a patient of our practice, access and use our website, or communicate with us through writing or phone. We generally collect your personal information directly from you unless it is unreasonable or impracticable to do so. This may include information from your guardian or responsible person, other healthcare providers directly involved in your dental care such as specialists, your private health insurance, Medicare and other government funding bodies (e.g. Child Dental Benefits Schedule, Department of Veterans’ Affairs).

We may collect personal information including your name, date of birth, address, contact details, family contact information, dental and medical history, private health insurance information and Medicare number.

When you access our website, we may collect anonymous technical information about user activity on the website such as the browser type and the pages visited. We use this information to maintain and improve the user experience of our website.

Disclosure of personal information.

We may disclose your personal information:

• to third parties who work with our practice for business purposes, such as information technology providers
• to other healthcare providers directly involved in your dental care, such as specialists

Personal information is destroyed when no longer required to perform our functions, except where it is retained for longer periods to comply with legislative requirements for document retention. We maintain computer and network security by various means including firewalls and user authentication to control access to our computer systems.

Access and correction of personal information.

You may request access to, and correction of, your personal information by contacting us in writing.

We acknowledge our patients may request access to their dental records. We ask that such requests be made in writing and we will respond within a reasonable time.

We take reasonable steps to keep all current personal information up-to-date, accurate and complete. We will confirm your identify before giving access to your personal information.

Privacy-related complaints.

If you believe that your privacy has been breached, please contact us in writing and provide comprehensive details of the incident so that we can investigate it.

We will treat your complaint confidentially. Our representative will contact you within a reasonable time after receiving your complaint to discuss your concerns and outline options regarding how they may be resolved.  We will aim to ensure that your complaint is resolved in a timely and appropriate manner.

We will also undertake best endeavours to prevent future breaches by making any necessary changes to our processes. If you are unhappy with our response to your complaint, you may then contact the Office of the Australian Information Commissioner, who may investigate your complaint further.
