We may collect personal information including your name, date of birth, address, contact details, family contact information, dental and medical history, private health insurance information and Medicare number.

If you believe that your privacy has been breached, please contact us in writing and provide comprehensive details of the incident so that we can investigate it.

We will treat your complaint confidentially. Our representative will contact you within a reasonable time after receiving your complaint to discuss your concerns and outline options regarding how they may be resolved.  We will aim to ensure that your complaint is resolved in a timely and appropriate manner.

We will also undertake best endeavours to prevent future breaches by making any necessary changes to our processes. If you are unhappy with our response to your complaint, you may then contact the Office of the Australian Information Commissioner, who may investigate your complaint further.
